The repeatable quest is available from level 20 inside catacombs and necropolises. Dimensional Fragment is consumed when entering Dimensional Rift.
Walkthrough

Accept the dimensional-fragment collection from the rift-related NPC inside an eligible catacomb or necropolis.

Hunt ancient-magic monsters whose names contain Lookout, Watcher, Guard, Monk, Overlord, Shaman, Consul, Inquisitor, Guardian, Elder, Court Guard, or Preacher.

Do not count Nephilim, Lilim, Gigant, or Lilith variants excluded by the journal.

Collect Dimensional Fragment and keep enough for the rift rooms you intend to enter.
