The one-time quest is available from level 61 from Hierarch near Evil Altar. You receive four Blood of Saint, one for each altar.
Walkthrough

Talk to Hierarch and receive 4 Blood of Saint.

Find an Altar of Saints and spread Blood of Saint through its dialogue.

Repeat the action at the remaining three altars, checking that the journal advances after each one.

Return to Hierarch after the fourth altar. The recorded reward is 105,527 XP.
