Preparation
The one-time quest is available from level 62 from Hierarch near Evil Altar. You receive five Crystal of Seal, one for each seal.
Walkthrough

Talk to Hierarch and receive 5 Crystal of Seal.

Find an Evil Altar and break its seal through the altar dialogue.

Repeat the action at the remaining four Evil Altar locations, checking that the journal advances after each one.

After breaking the fifth seal, talk to Hierarch nearby. The recorded reward is 221,958 XP.
Description
The Hierarch says that you must pass a trial to be recognized as a member of his group.
